NRHI Kicks off Support and Alignment Network Initiative

In October 2015, the Centers for Medicare and Medicaid Innovations announced awards to 39 national and regional collaborative healthcare transformation networks to provide technical assistance support to clinicians to improve quality and cost of care.  Transforming Clinical Practice Initiatives (TCPI) is one of the largest federal investments designed to designed to support clinical practices through nationwide, collaborative and peer-based learning networks that facilitate practice transformation.

NRHI was awarded as a High-Value Care Support and Alignment Network (SAN). NRHI will leverage work with RHICs and national partners to advance the goals of the Practice Transformation Networks (PTNs).

On November 17, 2015, a kick-off event was held to introduce the NRHI SAN to the PTNs included in TCPI. The NRHI SAN is working with the following Regional Health Improvement Collaboratives (RHICs) who will provide clinical and quality expertise to PTNS throughout the country:

  • Minnesota’s Institute for Clinical System’s Improvement
  • Minnesota Community Measurement
  • Pittsburgh Regional Health Initiative
  • Massachusetts Health Quality Partners
  • Maine Quality Counts
  • Oregon’s Health Care Quality Corporation (Q-Corp)
  • Better Health Partnership Northeast Ohio

Program areas include but are not limited to:

  • Understanding and using total cost of care
  • Patient engagement and experience
  • Patient safety
  • Shared decision making
  • Reducing unnecessary tests and hospitalizations through Choosing Wisely
  • Quality Improvement program design
  • Population Management
  • Having difficult conversations with patients and providers
  • Primary care and behavioral health integration

The NRHI SAN recognizes the value in working collaboratively and is coordinating efforts with the fellow SANs to provide consistent messaging and program offerings to clinicians and practices throughout the country. The NRHI SAN will leverage the RHICs to promote the NRHI SAN High-Value Care Learning Program and enhance collaboration and partnerships with local and regional PTNs.
