In this module of the Reducing Unnecessary Utilization series, Nancy Zionts, of Pittsburgh Regional Health Initiative, will lead a discussion on the ways to use POLST (Provider Orders for Life-Saving Treatment) to address readmissions. She will be joined by Judy Black, MD, of University of Pittsburgh, School of Medicine, who will discuss the benefits of using POLST in a practice setting as a strategy to reduce readmissions. Dr. Black serves as the Pennsylvania representative to the National POLST Paradigm Task Force.
