In this module in the Reducing Unnecessary Utilization series, Bruce Block, MD from Pittsburgh Regional Health Initiative will lead a discussion on the importance of collaborating with specialty and sub-specialty care services to reduce unnecessary hospital care.
You will learn about:
- The different opportunities and challenges that sub-specialists face in their efforts to assure quality and control costs
- Successful practice initiatives that sub-specialists have used to reduce avoidable admissions including care coordination and patient engagement strategies
- Approaches to building care collaboration to support sub-specialty care efforts
This module is intended for PTN practice advisors and practice staff participating in a PTN, and others focused on improving specialty access. This module will address the following TCPi change tactics:
1.2 Team-based relationships including 1.2.4 Define specialty-primary care roles; 1.4 Practice as a community partner including 1.4.4 Use Community resources; 1.5 Coordinated care delivery; 1.6 Organize, 1.6.5 Reduce unnecessary tests
