In this module in NRHI SAN’s Advancing Care Management series, Tani Hemmila, MS, from the Institute for Clinical Systems Improvement, and care manager, Todd Hinnenkamp, RN, will share what they have learned about “Powering Up” care management.
In this module you will learn about:
- The five components found in highly effective care management programs for patients with complex / co-morbid needs with a goal of improving population health.
- Lessons from the field, including implementation tips from the successful national COMPASS collaborative initiative
- Tactics and tools for moving from basic care management to ‘powering up’ population health management for patients with complex needs.
- How to partner with patients and team members in new ways to improve care management support
This module is intended for PTN practice advisors, nurses, physicians, social workers, practices participating in a PTN, and other interested staff, specifically, those focused on how person and family engagement can be used to support the following change tactics (1.3.4; 1.3.5 and 1.5.1; 1.5.3; 1.5.4)
