In the second module in NRHI SAN’s Advancing Care Management series, Jeyn Monkman, MA, BSN, NE-BC, Project Manager from the Institute for Clinical Systems Improvement, and Medley Shamp, RN, Care Coordinator Lead for Tri-County Health Care will share their insights and experiences on how registries can support and improve efforts on care management and population health.

- Practical strategies for using a registry to improve complex patient care while partnering with care managers and the health care team in innovative ways
This module is intended for PTN practice advisors, nurses, physicians, social workers, practices participating in a PTN, and others focused on how registries can be used to support the following change tactics: 1.3 Population Management, including 1.3.4 Develop registries and 1.5 Coordinated Care Delivery.
