Continuing our person and family engagement series, Nancy Zionts from Pittsburgh Regional Health Initiative (PRHI), will discuss the changing field of end of life care and how to support your community and healthcare providers to engage in difficult conversations around life limiting illnesses to effect appropriate advance care planning decisions.
In this module you will learn about:
- How chronic disease and end of life care are changing in America
- The disparity and disconnect between what care people expect at end of life and the reality
- Educational tools and opportunities to improve professional’s skills to engage in difficult conversations
- Strategies for community engagement around planning for serious illness decision making, including recommended resources
This module is intended for PTN practice advisors, nurses, physicians, and social workers and other staff, as well as practices participating in a PTN; specifically, those focused on how person and family engagement can be used to support Change Tactics related to Person and Family Engagement (1.1.3; 1.6.1; 1.6.2; 1.6.3; 1.6.5)
