In this third in a series of four modules on person and family engagement, Barbra Rabson, President & CEO, Massachusetts Health Quality Partners (MHQP) and Amy Stern, Senior Project Manager, MHQP, will be joined by Dr. Michael Yogman of Yogman Pediatrics, and Lindsay Gainer, Senior Executive Director of Innovation for North Shore Medical Center and North Shore Physicians Group to offer recommendations for working with families and practice care teams to increase person and family engagement.

This module is intended for PTN practice advisors and other staff, as well as practices participating in a PTN; specifically, those focused on how patient and family engagement can be used to support Change Tactics related to Person and Family Engagement and Continuous, Data-Driven Quality Improvement.
